Marital name change but not on passport for PR application

New here, new post, so many questions to ask, hopefully I get answers before next week when I intend to send my application.
I am about to send in my PR application by September 1st 2012(next week), I am going to be sponsored by my husband. We got married 2 months ago in June here in Chicago and I have currently and legally changed my name to his last name here in the U.S(Drivers License and Social security), however I have not changed my passport yet, since it expires in 2015 and it is not entirely a quick process(a Nigerian passport) to change this since we intend to apply for the PR as soon as possible.

So my question is, what name should I put on my application? Some parts on the application form specifies that I have my name as it appears on my passport, but other parts of the application doesn't specify that. What would be the repercussion of not changing my passport to my new marital name? Would my PR card come with my maiden name or is there a way I can specify that I would like it to have my new marital name on the PR card even though my maiden name is still on my passport?

I ask these questions because I wonder if my PR card shows up with my maiden name I would be technically stuck with my maiden name in my future Canada license, health card and other forms of identity even though in the U.S it has been legally changed.

What is the ideal solution or advice in this case so I can have my new marital name used legally when I land in Canada?

You should use the name on your passport... the PR card will come in that name. You can get everything you need Canada license, health card and other forms of identity just by presenting your CoPR passport and your marriage certificate so, you can have these things done before you even receive your PR card in the mail. Once you obtain a license or health card with your married name you can send in to have your name changed legally on your PR card. This is also the option I chose figured I have 8 years left on this passport why am I paying for a new just to change me name once I have PR card thats all I need to return to Canada.

Legal change of name document

If your present name is different from the name printed on your Record of Landing (IMM 1000), Confirmation of Permanent Residence (IMM 5292 or IMM 5688), you must send in supporting legal documentation as proof of your name change:

a certified copy of a legal change of name document, court order, adoption order, marriage certificate issued by civil authorities with your new name or a divorce decree with your new name, and
a certified copy of your provincial driver's licence, provincial identification card or provincial health card in the requested name.

I have a similiar situation, since im double citizen, swiss/german, but i only got a swiss passport. I changed my german ID though, so in germany i go with a joined name of me and my husband but for switzerland im still with my maiden name, cause it would take too long as well to get a new swiss passport and the german ID i was able to change fast and since i know i wont come back so fast to germany and dont want do it by mail later, so i did it now already, easier that way for me. My swiss passport ill do after the application is through. But ya in the forms it tells you to put your name as it appears in your passport, so you do that! I did as well, i put as it is in my passport, means with my maiden name and i also used my maiden name everywhere on the application. And we married in canada and they have me with only my maiden name there too and i will changed it to the joined one later after i landed.

So in short.....use the name that is on your passport that you use to travel and to land. Thats important. If you change your passport and name between, you need to notify them of the change and send a copy of the new passport.
If you want to change your name after your landing you can do so by showing the (translated) marriage certificate to offices and insurances etc., your passport can be changed at a consulate/embassy of your country in canada too. I got a swiss consulate near us where im gonna live in canada and will then change my passport there.
Either way you wont be stuck with your maiden name on any ID's or your PR card, its always possible to change it after. Just cost time and money like anywhere else too :p

I think I am just going to keep my passport till it expires in 3 years. As long as I can have my drivers license, health card, bank info and my work related info in my new marital name by showing my marriage license, I think that will be just fine. Thanks again!
