Hi,

I came up with three question while filling up my form. Hoping someone can provide some feedback on this.

1> I traveled for 3 weeks for work in Sweden in 2015, which is within 5 year eligibility period. So should I add that information on question 10.A. ? I actually don't remember the hotel name and address that I stayed in.

2> After landing in Canada, I was unemployed for three month. So, what should be the explanation for that time period in Quesiton 11.

3> Being a resident of Ontario, do I need to check NO on question 17. C. Or just leave it blank?

Thanks in advance.

Hey there,

1. You don’t need to include addresses of family, friends, hotels or resorts where you stayed while on vacation. Question 10 is where you list your permanent addresses, so while you were traveling for 3 weeks, you would've had a permanent address somewhere. That's the one they'll want to see.

2. You would just say "Unemployed" in the third column for that period. That's one of the valid options.

3. I wouldn't leave it blank, just to be safe. While inclusion in the Quebec list is not mandatory, the question is not listed as optional, so I would just check the "No" box.

You only need to send the bio pages with your application.

During your test/interview they may or may not ask for the translation of your stamps. I was asked to get my stamps translated after my interview and that’s why I suggest having them done to take with you to your interview. It will save you time if they do end up asking for them.

Hey, one more question for ya: the two stamps in my passport that are in Chinese are from before I became a PR, so they don't affect my physical presence for the 1,095-day requirement. That trip was not even listed in my physical presence calculator. Do they look at all the stamps in your passport or only at the ones after you became a PR?

I had to get them all translated (even the ones before my 5 year time frame). Think of it like this, the stamps are in another another language so they can’t read the dates on those stamps and therefore can’t be sure they were from before you became a PR. Hope that helps!

Thanks a lot. These makes sense. +1.

Okay, thank you! Interestingly, the stamps are in Chinese but the dates/numbers are in English. But I'll get them translated anyway juuust in case.

Some additional question regarding the main form -

1> Since I don't have any other name. Should I leave the Question 5 - (List all names you have used) blank or put 'None'?

2> My passport expired last year, and I haven't renew it because I don't have any plan for travel. Should I put this explanation on the Table B for Question 14. Or do I need to write a formal application letter explaining the expired passport.

3> How to print the form. Print on both side or one side of the paper?

Thanks in advance.

1. Only list your name and choose "Name at birth" in the third column. You can't leave it blank.

2. Yes, list your expired passport and then use the box in Table B to explain why you don't have one at this time. They need to know all the passports you've had during your 5-year eligibility period. Any gaps in time should be explained in Table B.

3. While not explicitly mentioned anywhere on the guide, it has been highly recommended by members of this forum to print all forms single-sided.

Hi All,

For the personal history, CIC says this

Do not leave any gaps during this period and do not leave this section blank. If you do, your application will be returned to you.

My personal history looks like this

2016-05 - Present
2016-01 - 2016-05
2015-06 - 2015-12
2015-02 - 2015-06

For the 2nd entry, is there an issue with missing dates? Should the starting month of the entry always be the ending month of the previous entry? Not sure

Hello guys, though I am not for the citizenship. Actually on a work permit. Been told to go for my medical examination in one of the approved facilities in lagos. Can someone please direct me to a location and the amount it will cost me to do it? Thanks..
