I think you should mention that - just my take on it
Though most of Qs talk only about 5 year eligibility period, all previous name change should be still mentioned

Mention seems to be required - that's clear, but what about certified translation of the document and bringing this to application. This may not be so easy, considering that this one is quite old. Also, not clear - all photocopies (i.e. passport, diploma,etc.) should be cerified by notary public?

No document needs notarization (unless its not in English, then it needs notary & translation to english)
Copies are fine. Originals will be asked during the Test/Interview

We just attached a copy of the marriage and birth certificate of my wife with our application.
