All my 4 previous employers have mentioned my job title, hourly wage rate, full-time but they did not mention any job duties and responsibilities. Is that a problem? They are hospitals. My employers are based in USA. I have my tax forms, pay slips, contract letters etc. Will that be sufficient?

Yes, if no duties are specified in the job letters, you can probably expect a refusal.

If you can't get a reference stating your main duties from your employer
it is very important to submit your contract letter which I know will detail
your main duties,hrs of work and wages.

Write a covering letter by making reference to your contract letters stating
that those missing main duties in the reference are stated in your contract
letter.In this covering letter mention the phone numbers and email of the
contact person they could reach within the organisation.

One of my friend did this and CIC never queried her because she presented
enough proofs of working in the establishment.

This should be ok.
