We did our medicle from Dammam, KSA so no idea about IOM, you can check their number from DMP detail on CIC web site.

That's a great news. On which date the note was written by visa officer. You should get MR by maximum by a week from that instruction date.
However, make sure that your consultant is sending a CSE. Consultants are very lazy in sending CSE. You can also sent CSE by yourself referring GCMS notes with polite language.

3rd line on Ecas, time line is not constant for every applicant, u can take an idea from Mr. E's spread sheet.

Yes your consultant can request no requirement just money although it charges 5 dollars and your consultant may charge you 50 dollar for that or 100 dollar.........yes you can order as much as you can....

You can find visa officer's comments positive or any doubt, at the end under notes. interview exemption also can be mention there.
My advice ask your consultant to send CSE referring GCMS notes without delay.

Take an appointment with your local DMP. Stay healthy during this time. If you are smoker, stop until medical done.
Medical examination will happen on blood, urine test and chest X-ray.
Take passports, photographs and medical reports if you are taking any medication for the test.
At the end they will ask few basic health related questions.

Just be tense free and relaxed.
