Just have a question to clear my mind, do I need to get any form from the hospital after medicals as evidence that I had the medical test?, or just the reciept of payment ?, the hospital only gave us the reciept of payment with the tracking number. do I also need to send the receipt to LVO together with the other documents requested?

Please someone help me with this.

Hi,
you do not need to send any extra document regarding medical, the doctor will send everything. Just keep the receipt for your personal record. Just send the other documents as LVO asked.
