Based on your posting you were only in Canada for 300 days when you submitted your PR card to be renewed in December 2018. Then you mention you left because your mother is sick. Are you now in Pakistan? How do you plan on returning?

yes in Pakistan, someone suggested that i have valid USA visa i can travel by road via USA and show them supporting documents from my legal advisor. and i can also apply for PRTD with explanation of humanitarian grounds. that why i was low on residency obligation and why i am not there.

You can try to apply for a PRTD or if you get a US visa you can try to use your expired PR card to enter and attempt to prove that you you have H&C reasons for being gone from Canada for 4 years.
 
You should expect that your spousal sponsorship will be refused if IRCC discovers you have been out of Canada.

I think that you need to be working very closely with a qualified Canadian immigration lawyer.

You are now at the point where you are liable to lose your PR status and your sponsorship application.

You don't currently meet the RO. It seems that you didn't meet the RO for PR card renewal and you have not complied with the sponsorship residency requirement.

You are still at risk to lose your PR from your renewal where you never met the 730 days. You also don’t meet the residency requirements to receive healthcare since you have been out of Canada for so long.
 
Just realized that you are likely having an appeal hearing for your spouse in September. You really need a lawyer.
