Below just download this form from this link, at the second page there you will find the info for the lost passport. You were the LC1 with the LC2 who just recently arrived right? Well i hope this will help. Here's the Link

How about damaged passport?
I'm confused with the process I found on the net.
I have the form and all but whats the process. (Please correct me if I'm wrong / or add what I'm missing)

1. Affidavit with Notary stamp sa Ph consulate Toronto (Fee of $33)
2. Submit the application and supporting documents
3. Pay fee of $81
4. Wait for 10-12 weeks.

Also , I have a 10 year multiple entry visa for US within that passport. Do I apply again or will they just replace it? Any idea if any fee?

If it's easy for you to personally visit the Philippine consulate in Toronto, you might as well do that to get clearer instructions.

If the US visa page is not damaged, you can still use it. If the visa is damaged as well, you will have to apply for a brand new visa at the US consulate for $160.
