1) The PPR request is sent by the visa office or CIC or CIO ?

2) When we order the GCMS notes , for example from visafile. info .... They send the gcms almost after a month ... so when do they apply for our gcms notes ? I mean for e.g. if you sent them an email on 16 Jan ... then you will receive your gcms notes on 17 Feb ... But within this timeframe , when the GCMS is actually generated ? ?

CIC Official statement regarding the time it will take to receive PPR after medical results are received and approved by CIC:-

How long will my application take if I need a medical exam?
After a medical examination by a Panel Physician, the results are sent for review to the CIC medical office which serves your region. The CIC medical office will assess the results and send their assessment to the visa office which is processing your application.
Processing times can vary but generally:
For temporary resident (visitor, student, worker) applications, it normally takes four weeks after your medical examination before we will contact you regarding the next stage of your application.
For permanent resident applications, it normally takes three months after our medical office receives the medical examination results from the Panel Physician before we will contact you regarding the next stage of your application. This is true only if you have already complied with our other requests (e.g. to submit other documents).

2) it will be generated for the date it is applied by Visafile.Info (16 or 17 Jan) however the file will be received after a month. I went through this myself.
 
So which means that , whenever we apply for the gcms notes , within a couple of days gcms notes is generated but takes a month to be delivered to us
