In Process is not the full status.
Click on it and you will see either "We received your application ....." or " We started processing your application ...". That's the status.
As you have received MR, your ECAS should show "We started..." by now or within 2-3 days.

I am not sure whether they are encouraging to pay the fee online or by any other method. Does LVO accept RPR fee online?

Yes, LVO recommend paying online.
It is much easier and hassle free comparing to DD.
Just follow the below link and pay online. Then send the PDF receipt to them by email.

And no, there is no sequence, you can pay RPRF before medicals or vice-versa.

I completed my medical exam on 7 November. The hospital informed me that they uploaded the results on 11 November. However, they designated the medical office as Manila instead of London (I think, my application is processed over there). Should the ECAS be updated after they received the medical results? Would hypertension cause any problems with the delay in ECAS updates?

Would you explain what do you mean by "they designated the medical office as Manila instead of London". Who are 'they'? The hospital?
Does medical center need to know your visa office? I believe they upload it to eMedical system using the IME number.
