This refers to your application for permanent residence in Canada.
We require the following to be sent within 60 days:
-updated Schedule A and IMM5406 forms for you and your spouse **questions 8 and 12 of the Schedule A forms must be completed from age 18 and must not show any gaps in the dates. These forms may be downloaded from our website www.cic.gc.ca**
-medical examination results for you and your family **instructions will be emailed to you separately. A separate form will be sent for each family member. If you have not received them within 48 hours, please contact us**
-updated Qatar police certificates for you and your spouse **we assume you are still resident in Qatar. If you are not, then please advise us. Details of how these may be obtained can be found on our website www.cic.gc.ca**
-Right of Permanent Residence Fee **bank draft for CAD 980 payable to The Receiver General for Canada is required**
Note: the updated Schedule A and IMM5406 forms are required for mandatory background checks, therefore, please forward them to this office as soon as possible. You may scan and email the forms to us if you wish. The other documents requested above should be sent to this office by mail/courier.
Fill, print, sign and scan the updated Schedule A and IMM5406 forms for you and your spouse and send it as soon as possible by email.
