i haven't heard of anyone paying processing fee for addition of wife by submitting "payment fee forms" to LVO. Majority of people paid theirs as bank draft.. one guy paid using online payment (based on what i have come across), the link for online payment has been posted earlier..
If I were you, I would send LVO an email and inform them that you have send the fees as filled in "fee payment form" to be charged on credit card and ask them whether it is acceptable form of fee payment. If not request them how you can pay the fees for addition of spouse.

120 days is what i was told to receive MR and its been 134 days since PER and neither ECAS 2nd Line update nor MR from LVO, so i suggest you just forget about any update from LVO in near future. normally IMO gives appointment of around 7 to 10 days gap. Cost of Medicals is approximately PKR 7,000/- payable in cash.
