I think it depends on your status - RBVO or IN PROCESS - ( but some times online status is misleading )
if it`s in process ,and you are sure that every thing in ur document is O.K , then expect medical soon , most applicant receive there medical within months (1-3 months) after in process , but always there is exceptions

but if ur case is transfer from isl. office to London office , then I don't know about it , because i`m not from Pakistan

dear zakout, if your status shows RBVO it means:Received By Visa Office. This means your full documents have been received at the visa office and are waiting to be processed. After 2-3 months(time varies, may be longer) the status will change to In Process: this means your file is undergoing reveiw by a Visa Officer and at this stage he is likely to ask for more documents from you, if he is not satisfied with all that you have sent. If everything goes well then at this stage he will send you medical and RPRF request. So normally during RBVO no documents are requested from you BUT sometimes the VO may not change your e-case status to in process and may start reveiwing your file, so your status is actually in process but you dont know, you think it is still rbvo-then you may get some requirements at this stage(that is how some forum members will tell you not to trust e-cas too much and be prepared at all stages). all the best and dont worry everything will work out just fine.

you are follow the old system (before 28 Feb 2008) which I have no experience in it .
but according to www.trackitt.com most of 2006 applicant and about 30-40% of 2007 applicant got visa

"RBVO" mean only receiving your document , so mean nothing for processing

"in process" mean starting processing your file , then if there is any missing documents , they will ask for it
if every thing is O.K then medical will came

but some time they didn't update e-case

according to my experiance those guys who send addt documents in jul/Aug and sep got their meds. so u plzz send one more case enquiry or the best solution is applying for CAIPS. so u can clearly undersnad where u stand
i am a guy who send addt documents in July and got my meds in sep
