LMO Required to extend caregiver contract with same employer?

The caregiver that I hired from outside Canada and is with me now was given a one year work Visa. My question is: Will I need to submit a new LMO for him to apply for an extension. Can he ask for a 18 month or 2 year extension? Will CIC Kelowna accept his extended application or does it have to go to Vegreville? The CIC web site are not clear on this.

I just found Operational Bulletin 025 –
January 3, 2007

at this web address:

which to me says that a caregiver applying for an extension for a work visa with original employer does not need a new LMO and can apply for the remaining time of 2 years plus 3 months after the first one year permit. Is there an updated operational bulletin?

I called HRSDC and I was told that the expiry date on the LMO is not used anymore. Because of the economic situation an LMO is needed for every extension, yet when I called CIC, they said LMO only required when worker applies to a new employer. Confusing isn't it?
