If I have only one name that I have used all my life, do I still have to fill this question?

I filled the only name of mine in this question and put it as name of birth. But not sure what I should do for the other blank rows. Should I leave those two Rae's blank or put N/A?

There is a question mark next to that question, please click on that. Most people put NA or select name at birth from the drop down and write your current name

I am also struggling for this question, from my own understanding, i can leave it blank since i don't use any other name officially after birth.
However, seems people prefer to put name at birth in this question to make it safe.
