Hi, I also received AIP without medical forms. I'm not sure about the next step to take. I read that people got their medical forms with their AIP. my child is also back home.I'm African.

Then separate email for each dependent for the medical form with the barcode is also an attachment for each of us. All sent/received the same day. No medical for hubby because he is the LC1 ;) correction: aip is 2 pages and email with 5 pages attachment includes the instructions for LC2 & LC1

you may try to phone cic call centre and request for the med forms to be sent to your email.

she can used her birth certificate or school id. i have non accompanying minor dependent too.
 
To those who had their medicals done & with dependents. what if you're children do not have immunizations after 1 yr old but they are now 14 yrs old and above already, how many shots did st lukes give them for catch up? how much is the additional fee? thank you in advance for those who will reply...

i called them a week ago and have not get it yet. i didnt call right away thinking that the letter was good enough to take the medicals until my husband called the clinic and was told that there should be med forms. Has anyone tried to go to medical without the request form?

Thanks Margaux.I think you should call CIC call centre again for the med form. I will call next week.

I am in Cebu and had my medical exam done here. I am not sure about any shots needed for minors but for me I was only given a shot for measles. In Cebu the medical exam package is 4100 plus 1,500 for the measles shot.. so i paid a total of 5,600.
