Forms

If an officer cannot establish that your family members meet the requirements for permanent residence and that you and your family members are not inadmissible, your application for permanent residence in Canada will be refused. Medical Examinations All your dependent family members, whether accompanying or not, must undergo medical examinations before you submit the required documents. Police Certificates Your family members, 18 years of age and older, must obtain original police certificates from each country or territory where they have lived for six consecutive months or longer since reaching the age of 18. The police certificates must be sent together with the forms and documents you are submitting. Do not send police certificates separately or they will be returned. Right of Permanent Residence Fee We strongly recommend that you pay and submit proof of payment of the Right of Permanent Residence Fee with the complete application. As an applicant for permanent residence under the Live-in Caregiver program, you are required to submit additional information regarding your family members.
This document checklist outlines the forms and documents you must gather, where applicable, sign and submit for your family members.
When you are submitting the required forms and documents to this office, please include a copy of the letter and this checklist.
The forms can be obtained at: http://www.cic.gc.ca/english/information/applications/live-in.asp . For completion of the forms, please follow the instruction guide on the website, but please note that you, not your dependants, will be required to sign the forms you submit for your dependants. You are responsible for the truthfulness of all of the information on the forms you submit.
Gather the documents for your dependants in the order of the checklist and check each item. Attach small items such as photos with a paper clip, otherwise leave documents unbound. Place all documents in a sealed envelope.
If your supporting documents are not in English or French, they must be accompanied by the English or French translation completed by a certified translator and a copy of the original document. Do not send any additional documents when submitting your application to the Case Processing Centre in Vegreville.
Note: If your submission lacks any of the documents listed in this checklist, your application will be processed based on the information on file. If an officer cannot establish that your family members meet the requirements for permanent residence and that you and your family members are not inadmissible, your application for permanent residence in Canada as a member of the Live-in Caregiver program will be refused.
