PM your email and i will send you an excel format. Data are from your pay slip. After filling out the form have your employer sign it and attached your pay slip. LOL! i didnt realize that is passport sized photos in the checklist.. i followed the photo specifications on the pr guide..and i brought that guide to the photo studio and the lady asked me if it's for pr app, i told her it's for pr app... now, you're making me confused.. hahaha!
