When I was in my home country I used my last name un-expanded. Let's say my full name is "Betsy Foo", then back home in all official documents my name is "Betsy F".

Later when I got my passport I got it with my full name "Betsy Foo".

So long story short - In my 4 years bachelors degree, and my first 2 years work experience as a software engineer back home, all my documents show the name "Betsy F". I then came to US and got a Master's and PhD, and some work experience while at University. All these documents have my passport name - "Betsy Foo".

So my questions:

1. Do I need to evaluate my Bachelor's degree with the name "Betsy F". I am getting my Masters and PhD verified by WES.

2. If I want to use my two years work experience from back home, then the reference letter from the company comes with the name "Betsy F". How can I explain that to CIC?

1. WES will issue your ECA Report as per the name mentioned on your educational documents. However, you can write to them with copy of your passport saying that the F in your name stands for Foo. So WES generally accepts such requests and would mention Foo in brackets.

Hi Rusticdud,

I have the same issue as yours. My passport has "AAAAA BBBB", but my bachelors and systems engineer work in India as "AAAA B". I got the one and the same person affidavit as mentioned by Stanlee. You can modify the below template.

Please find the sample affidavit format. I believe you can get the affidavit from lawyer or oath commissioner.

AFFIDAVIT
ONE AND THE SAME PERSON

I, the undersigned, Mr. AAAAAA BBBBBB, aged 29 years, residing at Ontario, Canada, do hereby solemnly affirm and declare on oath that:

1. I am the deponent herein and a citizen of India.
2. I was born on X, 1986 in City, State, India.
3. My birth name is AAAAAA (First Name).
4. My father’s first name was mentioned as initial in some of my documents.
5. Therefore, the following names refer to the one and the same person:
a. AAAAAA (First Name) BBBBBB(LastName)
b. AAAAAA (First Name) B(First letter of my LastName)

The above are the variations of the same name and should not be considered name changes.

So I do hereby solemnly affirm that foregoing are true and correct statements.

If i am in the USA, would you happen to know if i can get this affidavit signed by any notary (say at my bank here) ?
or do i have to go to a private lawyer or somewhere else ?

I recently got affidavit for a separate purpose from a UPS store. It is very easy, just go there with the self declared document and sign in front of them.

Hi,

My wife is the primary applicant and we got our ITA on 19 October 2016. Our application was in my wife's maiden name as we were in the process of changing name in all the documents. We received her new passport with her new name and new passport number on the day of ITA. Now, the ITA has been issued with earlier details.

Can someone please help us with -
1. Whether we need to change her details in the application? If yes, at what stage?
2. Also, we are unable to locate the 'Permanent Residency' tab (mentioned in the ITA letter) to generate our personalised checklist and begin with this process? Is it under 'Continue Application' tab?

FYI - I was advised on another thread, that in my case the answer would be 'No' for this question. I am proceeding accordingly.

1. You need to update her name as per passort in the forms that are shown to you after you click Continue.

2. Click Continue, fill all the forms and at the end you will have the personalised checklist.

Do I need an affidavit in the following situation?

My name is X Y and my father name is Z. In all my education documents my name is written as X Z Y, because by our education law the father name should be included in the diploma and transcripts.

In all other documents the name is just X Y.


Is an official Birth Certificate (in english), where my father name Z is mentioned, apart from my name X Y and all other details, enough to prove my point?
