Regarding permanent resident card application form for the first time, im a bit confused of questions 19 and 20, your address history for the past five years or if you become permanent resident less than five years, your address history since becoming a permanent resident.

My question, as first time applicant do i need to fill up this question or just put NA. Thanks for your help.

why do you need to fill out the "permanent resident application form"? If you are landing for the first time you dont need that form,
once you land you just need to give the COPR & Passport to the immigraton officer and he will fill out COPR and will forward the new PR card to the address which you provided.

Page 1 of this thread says this form should be filled up by us. Thanks for clarification that it should be the other way around.

That page is written in March 2010... Now, It will be filled automatically by CBSA officer at airport

When I will bring my personal stuff (the time after landing) what I have to declare to the border about my personal stuff?

During the landing I count to bring some gifts (puppets, candy, jam, etc) and also my personal clothes etc... I have to compile the forms Goods Accompanying List and Goods to Follow List???

Will these forms be filled the second time that I will entry in Canada definitively?

I did the landing exactly one month ago (Toronto airport).
I spent only 10 days in Canada (at the border I declare that I wanted to activate my PR and I had planned to stay in Canada for only 10 days).
The procedure was been, more or less, the same that it is explained in this thread, the only difference was the "good to follow" step.

In fact, the last landing step was the "good to follow" procedure; the officer (very gentle) told me that it was not necessary to present the "good to follow" module in that moment because I was not in Canada to establish but only for a visit. I asked him the confirm for three times, and he responded yes all the times.
Furthermore, he told me to add to the module the Electronic device Serial numbers, and in case of they were OLD (without serial) write just OLD

At the end the "good to follow" module must be presented only the first time that you intend to establish in Canada (that, as my case, can be different from the landing date).

Application transferred to VO London on 30 June 2010,

Application reviewed at processing started at VO London on 24 January 2012.

It's almost 16 months now so when should I expect the medical request?

Isn't it late?

About 47 months have passed since the intake office received your application.The average time for processing is currently 38 months for LVO. It is definitely late and you should contact the VO through a Case Specific Enquiry for an update. Hope it produce some positive outcome quickly. Good luck.
