I dont know; its true that is nowhere mentioned, but to sign any statement you make is a legal principle. It doesn't make sense. The additional sheet is considered as an additional page or item. Otherwise, how about the remaining pages of the form which don't have the signature field? In the application form, I just mentioned 'Please refer the additional sheet' in the travel history section.
In the additional sheet, I have mentioned the section number and the table containing my travel history. No signature in additional sheet.
It was accepted and I got my PR card renewed.
