Yes, you should mention the UCI and Application number. Also, don't forget about your wife's name, date of birth, the photo studio's name, address, and the date the photo was taken at the back of the photo.

Application sent June 1
Delivered June 5

Called in this morning(july19) to find out that the application is being sent back due to lack of proof of relationship.
Thats all the agent mentioned, now have to wait for Canada post to deliever our applicaion back. (Does anyone know how long it takes for it to come back)

So the question is, once I get the application back in the mail, should I refill all the forms or just update whats missing and send it back?
What are everyones thoughts?

You should check if you need to redo the checklist (there's a June 2017 version), since submissions using the old one were only accepted till 15 July.
