Since you will be signing this form it may be best to use your current info but also include your previous employer too. If I remember correctly there is space for previous employers. Have you included your record of employment from both ? I think its a good idea to include that record.ultraismo said:I messed up with the Option C printout.. Since they are asking for information from the timeframe when application was received, should I fill in the 5481 with the info from that date (June 2014) and not current info (have changed employer since then)?

Do you have your record of employment from both places? Make sure you photo copy this too in case its lost some where along the way.
