hi everyone!
I just want to ask where I can get the form of medical 1017B UPFRONT
my husband received letter last july 9 from missuaga with the UCI and application number but they are asking my medical 1017B UPFRONT so I went home last Thursday to do my medical but they are asking the referral letter from visa office or client biodata in order to do my medical since our application already submitted it confused me where I can get that form and how come the visa office never sent us?

Meron akong friend ganyan dn ngyre dahil hnd nla sinabay medical sa pag pass ng application. Tawag kayo st lukes ermita, mas alam nla process dun kesa dun sa makati or st.lukes global.


Yung isa ko dn friend kc tumawag sa makati sbe nung sumagot kelangan daw ng referral letter. Hnd nla alam kung ano ung upfront. Tpos nung tumawag sya sa ermita, alam na alam nla ung procedure.

Yes, just show up sa Ermita. Alam nila kung ano yung upfront. Dala lang ng ID/Passport.

hi thanks sa info tawagan ko ang st. luke bukas para makauwi din ako sa pinas to do my medical worried kasi ako last Thursday umuwi ako kasi my appointment ako sa ermita ang sinabi nila di daw cla pwedi gumawa nang medical ko kasi naka submit na raw ako nang papers ang visa office na raw gumawa nang client biodata ko kahit pinakita ko iyong papers ko requested to do my medical soon as possible.
