Why is it? should there be like "Decision has been made on your application ~~~" added on the forth line?

my time line is

AOR recieved 2014 September 30.
Medical Requst 2015 December 19.
RPRF request 2015 December 21.
Medical Results have been recieved on ECAS 2015 December 31.
PPR- Visa Exempt Country.

Hello I am July 22nd 2014 applicant.
Here are my timelines,
• AOR received 2014 September 25.
• Medical Request 2015 Aug 27.
• Medical Results have been received on ECAS 2015 September.
• RPRF request 2015 December 14.
On 2016 Jan 11, I got a mail from CPC and asking me to send a new Schedule A Background Declaration form (IMM 5669) along with police certificate.
It is fine they are asking for My police certificate, since it is already 1 year over after submitting.
May I know why they are asking for the above form again? Is the back ground verification is not over yet? Is there any other reason for it?

Have you left Canada since you filled it out the first time? Switched jobs, moved, etc? Perhaps it was filled out incorrectly the first time? My boyfriend had to re do one in November and we don't know why either. He did leave Canada last March for a 2 week period though so that could have been the reason.

I had applied from India and visited Canada for 3 weeks on a short term visit last year.
I will come under outside Canada category and currently in India.

I have the same - appeared about 3 weeks ago. Lines 2&3 the same, just a day out?!
I cannot be bothered to try to get through to a call centre agent to ask - as I doubt that would be useful or informative.

Hi Andy, they never asked me for additional documents !
