My panel physician was Dr Chan on Broadview & Danforth. Google him and call to book an appointment. Depending on how busy they are you could get in same day or take a slot for a few days later. I was in and out in 20 minutes - give medical form printout, show passport, pay cash ($165 iirc? The lady will tell you on the phone), sign form, take webcam picture, get urine, standard health checkup, medical questions, 1 vial of blood drawn. Get x-ray form and directions to walk 5 mins up the street to an X-ray clinic. At x-ray clinic, give form and show passport, pay cash, wait for turn, take off shirt & take chest x-ray, then go home. I was surprised how fast it was.
Note that Dr Chan isn't your typical feel-good family physician. You are a case to be processed as fast as possible. He doesn't do small talk. You go in, follow instructions, he gets the info and medical data he needs and NEXT. Some people find this rude or offensive but I like his efficiency.
I asked and was told emedicals are usually uploaded in around 10 days. It's pretty much how fast the blood/urine lab sends them back the results, sometimes they're quicker, sometimes they're backlogged and slower. Out of the dr's control, luck of the draw. The only influence you really have is when you actually walk in for your appointment. So the recommendation is just book it and get it done ASAP.

You are not looking at the updated spreadsheet.
Here is the link to the correct spreadsheet:
