If you are planning to have your credentials evaluated by ICES, they have a page in the application package for evaluation wherein there is a list of organization , the type of report they require and the documents required by the ICES.
In my case, since I am not sure if my application will be successful, I just need the evaluation as a supporting document for my application. So, i checked the CIC as the organization and CIC requires basic report only and the ICES requirement is that I have to send my original documents. If my application for immigration is successful and I decided to get a licence and be a registered dentist in Canada, then i will have to have my evaluation upgraded to comprehensive, pay the corresponding fee and I will need an official documents ( these are the documents wherein the institution where you graduated from are the ones who will send your documents to ICES directly).
If you plan to check the CLPNBC, then you need to have a comprehensive evaluation plus supplemental and official documents are required.
