If anyone is going there personally to hand in the passport then you do not need to put the passport/s and the additional information sheet plud the pictures in an envelop. they take all the docs and put in an envelop themselves and give you a ticket.

You don't have to send a receipt. Your RM sends the reports to your VO, thats the acknowledgement of fees paid.

If you meant medical receipt for payment I didn't sent it coz my medical report sent by the medical practitioners but show as a proof you better send the copy of medical receipt. To do the medical I don't know which country you from. I have done it London and it takes only 45 minutes. I have seen some of our forum members takes even half a day.
