Job Reference Letter... Whom should i ask for?

Guys, I have a questions about getting job reference letter for PR.

I'm working in pretty decent size corporation which they have HR department.
Whom should i ask for my job reference letter? HR or my manager?
Who's signature and contact information should be listed on job reference letter?
