I am currently on an 3 Year Open Work Permit and I have a Permanent Job which I’ve done for one year. Can I use my Proof of Work Experience letter as a job offer.

Job offer for what? You can count a year of Canadian experience but not 50 extra points for a job offer.

Does a job offer have to be a totally new job - TEER 0,1,2,3 or I can obtain a job offer with my current job title

Why do you need a job offer? It doesn’t give you extra points. To get Canadian work experience it must be TEER 0-3

Not for Express Entry since you are on an open work permit. Your employer must obtain an approved LMIA if you want to claim the 50 job offer points.
