I applied GCMS notes on 28sept 2020 but no reply from IRCC till date also waiting for medical, current application status bar is 65%. It's really hard to keep calm in this situation. Only one thing is in our hands that's pray to God...

1. Do I need to fill the form, print and sign, and then scan and upload it?
2. I dont have enough space for all details, so how to add the details in another sheet?
3. If I have left one job in feb and joined another in march, do I have to provide the one month gap too?

Yes you have to fill that out print it out and sign it. If you don’t have enough space add extra sheet of paper and put your name, DOB and uci on that sheet and fill all the history over there and then at bottom sign it. For the gap you can mention it as unemployed. Make sure you fill it and then print and sign.
