Anybody knows what number should I call to follow up my OWP hard copy its been more than a month that I've got approved and not received it yet.
 
You need to submit a medical *and* apply to have conditions changed - normally would take a few weeks to a month, these days, who knows how long it will take. You can't call and ask the conditions be changed.

I took a medical in September last year because It was one of the requirements for school and I submitted the e-medical information sheet with my OWP/PR application.

Do you think they are gonna give me OWP with the restriction?
I'm hoping it won't have the restriction but I'm not sure what I will get.

Provided that your medical was still valid, they should accept that and most likely issue you the OWP without the restriction BUT I am not certain.

SIN: You will have to wait for the OWP hard copy.

Ontario Health Card:

1) To meet the minimum qualifications you must:
  • be physically in Ontario for 153 days in any 12‑month period
  • be physically in Ontario for at least 153 days of the first 183 days immediately after you began living in the province
  • make Ontario your primary home
Also, One of the below:

1) have applied for permanent residence, and Immigration, Refugees and Citizenship Canada has confirmed that:
  • you meet the eligibility requirements to apply
  • you have not yet been denied
2) are in Ontario on a valid work permit and are working full-time in Ontario, for an Ontario employer, for at least six months
  • your spouse and any dependents also qualify if you do

Provincial health will depend on your province. In BC and Alberta you can get one. With Ontario PA needs to work for some months
