What are the amendment pages in a passport?

I think the amendment pages would be the last few pages (maybe page 27?) in American passports, and I think a name change or something would be a reason to have something on there. Mine were blank, so I just submitted the bio page (with the photo on it).

File transferred to CPC-M: March 19, 2019
Biometrics instruction: April 9, 2019
Ghost update: May 12, 2019
Decision Made: May 12, 2019
Passport Request: May 13, 2019

NO ADDITIONAL DOCUMENTS WAS REQUIRED.

IN OUR PASSPORT REQUEST, IT ALSO COMES WITH THE PRE-ARRIVAL LETTER ON IT AND THE APPENDIX B. THE APPENDIX B(IS NEEDED TO BE SUBMITTED ALONG WITH THE PASSPORT, 2PHOTOS AND THE PASSPORT REQUEST LETTER BY ITSELF.

NOTE: If your file has been transferred to CPC-M, they don't usually send any AOR2. You will just have an email regarding being transferred to them. And consider that as AOR2

hello..I recieved my ppr request last June 13..I mailed last June 18 with all the required information like photo the letter passport and the appendix B which I filled up with my information and my husband and 2kids..is this right? or it should only be me?
My family is still in phil so they have to suit their passport as well
my 2nd question is..I will send to them as well the letter and appendix b, now what information should be in the appendix B? is it only their name or all of us?
