This form for an application under subsection 5(2) is intended for minors (persons under the age of 18) who are permanent residents of Canada who are applying at the same time as their parent or who already have a Canadian parent. The application form should be completed by the minor’s parent or guardian.

Hello everyone
We are family of three and intended to apply after 18 January
I have couple questions and appreciate if you can help me with
1- We arrived in Jan 2015, so we have no taxes for 2014. In this case (my understanding ) is we have to submit 2015, 2016 and 2017 taxes to be eligible. Right?
2- We have been in another country for the whole year of 2014. So automatically (again, my understanding) we have to provide police clearance certificate along with our application. Right?

Yes, you are right;
1. you need to submit taxes for atleast 3 years in order to apply. So I guess you would have to wait until you file taxes for 2017.
2. And I guess you are in Canada for less than 3.5 years in past 4 years or in other terms you spent more than 183 days in past 4 years (from date of your application) in some other country, so you have to submit original police certificate with your application.

Actually, after using the physical presence calculator, it shows that by 18 January 2018 we will have 1096 days in Canada and we are eligible to apply

"Applications received at our Case Processing Centre (CPC) in Sydney, NS on January 11, 2018 or later must use forms from October 2017 or later (indicated on the lower left corner of the form). Application forms that are not up-to-date will be returned as incomplete. IRCC does not consider the date of signatures or mailing to be deciding factors.
Note: This does not apply to applications originally received at the CPC before January 11, 2018 and returned as incomplete. These applicants are not required to resubmit a new form versions. However, they must submit any missing information or documents the CPC requests."
