No need for ielts when you do ECA. IELTS is a separate assessment focusing on your fluency in english.you need to get score of 5 in all categories(Reading,Listening,Writing,Speaking)

for an over all idea, i would like to know that,should i do medical or biometric in advance and submit along with initial application?or should i wait for instruction from IRCC?

No. Biometric and Medical are to be requested by IRCC in the middle of processing your applications. And they have accredited hospitals/specialists for it in your country. You shoud wait for their instructions/request.

Thank you for your reply .so as per new application situations,applicant can apply for work visa for self and spouse,study permit for kids and PR application for whole family right?Everything goes in to same address of immigration services?Are you a nurse?
