This means the medical got extended.
If your medicals are expired and the online portal shows "you do not need an exam" that basically means you are waiting for a decision from an agent to either ask for a re-exam or just extending for one more year. But it is indeed strange they haven't decided on it since November. You should definitely try to contact them and ask about it. It can easily be that they have decided on it but not updated your profile yet.
