Hi KarmicYogi,

Once your received ITA, you will notice PR application with option to continue the application in your CIC account. Click on the continue, it will verify few things like (your name, passport , marital status, application payment )then, it will redirect to PR application forms like

1) Personal details (You would have already completed this section while submitting EE profile, now you need to update this section)
2) Contact details (You would have already completed this section while submitting EE profile, now you need to update this section)
3) Study and languages (You would have already completed this section while submitting EE profile, now you need to update this section)
4) Application details (You would have already completed this section while submitting EE profile, now you need to update this section)
5) Representative (You would have already completed this section while submitting EE profile, now you need to update this section)
6) Work History (You would have already completed this section while submitting EE profile, now you need to update this section)
7) Personal History (You need to fill in 10 years history)

Once you complete all the 7 sections, it will redirect to a webpage where you can upload your passport/Workpermit, Transcripts, experience letter and letter of explanation.

Hope this is clear for you.

Hi KarmicYogi,

I am not sure of birth certificate.

But no need of police certificate from Canada.

Refer the below link (http://www.cic.gc.ca/english/helpcentre/answer.asp?qnum=037&top=4)

Do I need to provide a police certificate for my time in Canada?
You do not need a police certificate for your time in Canada if you are applying for permanent residence from within Canada in one of the following categories:

live-in caregiver;
humanitarian and compassionate considerations;
temporary resident permit holder;
spouse or common-law partner; or
protected person or Convention refugee.
If you are applying under any other category or from outside of Canada, you may be requested to submit a Canadian police certificate for your time in Canada.
