I have the following queries while filling in the details for the ITA:

1. In the personal form do I need to mention the details of my parents and siblings not applying for PR

2. Does the travel history has to be only for the last 10 years barring the origin and the country where the person is residing

3. I have entered my wife's work details in work history though not claiming any points so should I remove it from there and place it in personal history rather than work history.

4. Where is the option to upload documents?
Once you fill in all the details and hit continue, you will get the page where the document have to be uploaded
 
How can I know where I will get my medicals done? there is no mention of any information anywhere in ITA except single message of ITA.details about instructions are not given. Thanks
 
I believe it used to be 60 and they increased it to 90. It seems some pages have not been updated to reflect the change, so they still say 60.

In your profile there is a counter that says "Days to submit" that starts at 90 and decrease by one every day, I would use that as the accurate one.

People , Plz help me to throw some insights on this issue
during medicals they found blood in the urine for my spouse (female). They asked her to come again after 10 days for the test to re check again. Can somebody tell me that this problem for my spouse will stop me getting a PR to canada

I believe you require PCC if you spent more than 6 months in a particular country. You need to add all non consecutive travels to see if you exceed 6 months periods. Eg if you spent 4 months in 2015 and then another 4 months in 2017, then you spent 8 months and require a PCC from that country.
