Actually I mailed to CPC Ottawa that I have not got MR yet ,even though I got RPRF which is already paid and RCMP(as requested) was submitted
Few mins back I got reply to my mail from CPC Ottawa.

Medical instructions are enclosed with this letter. Please open and follow these instructions carefully.
Please ensure that you note your Immigration Medical Examination (IME) number and UCI and present them to your panel physician.

Thanks but its general instructions like following ,no forms nothing.



Please bring the following to your appointment:

- Your Immigration Medical Examination (IME) number and UCI

- Proper identification (proof of identity must include at least one document with
photograph and signature such as a passport, driver's licence or national identity card)

- Eye glasses or contact lenses, if worn

- Any medical reports or test results that you may have as a result of previous or existing
medical conditions

- Four recent photographs, if the panel physician is not eMedical enabled (note: panel
physicians that are eMedical enabled are indicated on the CIC website listed above)


Note: It is important that you inform the panel physician of any previous or existing medical
conditions.
Failure to do so will delay the processing of your medical examination.
 
Same situation here.

Emailed CIC because I had already paid RPRF and sent biodata (both at their request) a month earlier but hadn't received MR. They replied 10 days later with the same email you received. It contained a PDF with instructions but no personalized form.

As this was the third time they were making a mistake when emailing me, I didn't want to wait some more for them to correct that mistake and went to the approved doctor (Francis Li here in Toronto). I was afraid I wouldn't be allowed to take the medical examination but the nurse was immediately able to log into my file and find all the information necessary, including my IME number (which I hadn't received from CIC but was mentioned as necessary for examination).

Thanks for your reply,so xray and all tests are in same facility or for xray you have to go to somewhere else.
Actually I am asking this question ,as I dont have car and I am going to go there from Brampton.

Also how many day they took to load your results.
 
Everything is done at the same place (actually in two neighboring offices - they'll send you back and forth). Total price is now 260$.

I did medicals a week ago. Results haven't been uploaded yet. Usually takes around 2 weeks. They supposedly upload on Saturdays.

Does dr Francis Li upload MR report electronically ? any idea?
 
Hello,

I had the same issue. CPC Ottawa emailed me MR without any attachment. I emailed them 3 times, no response. I mailed and faxed them too but still no response. So i decided to just go ahead with the medical exam without the form.

Can you tell me how long it took CPC to respond to your mail?

To be honest it is still mystery for us to identify this :)
No one can say any specific time line
