I am sorry if I offended anybody, but be reasonable, when a persons stands ankle-deep in water and ask for help to take him out of the water, I can only say "Why don't you step out yourself".
Here is the text of my "medical" letter:
To continue processing your application, a visa officer requires that you submit the following information and documents. Please submit your documents as a single package. In order for us to match your package with your file, please include a copy of this letter in the package.

- Medical examination. All dependants must undergo medical examination even if they are not going to Canada. Please present this letter to the doctor as well as your original passport. Upon completion of the examination you will be provided with proof that you have taken the examination, you need to return this proof to the embassy with your original passport. If you are currently not residing in China and will not undergo the medical examination in China, please advise us immediately so we can provide you with different instructions.

- If your mailing address is in China, you should provide us with four (4) address labels with your complete name and address written clearly in Chinese characters.
and "extra medical" letter for one of my son's:
Dear Applicant,
Your immigration medical documents have been received and are in process in this office. However, before we are able to make a final decision in your case we require some additional information.
Please take the enclosed sealed envelope to the physician who examined you within 90 days. The doctor will be requested to arrange for additional medical tests to be carried out and to send the results to this office.
As soon as we have completed our medical assessment we will immediately transmit our decision to the appropriate Immigration Office.
If you do not comply with the above instructions, your application may be refused.
