This looks like a certificate of employment and not a contract. A contract should state your salary and other benefits. What kind of document do you need exactly?

Most importantly the parties involved the contract , you and the organization.
 
Thx alot...BUT:
If THOSE INFORMATION SHOULD BE IN THE CONTRACT,THEN WHAT ABOUT THE LETTER OF REFERENCE???? In the appendix of London office they asked about the employment contract,then about the letter which must contains most of what u wrote sir....
Now, I have been working in the university for 3 years and some months......Do u think that I must provide the CHC with my first contract with the university?????Cuze the format I wrote in this thread is A CONTRACT RENEWAL CERTIFICATE........

When you sign a renewal of contract, doesn't the university give you a new copy of the contract itself??

If you can't obtain a copy of the new contract with the new dates then I suggest submitting the contract renewal letter along with your old contract.

A letter of reference is a document written by one of your supervisors or your immediate superior. It should mention your job title, job description and how they think of you as a worker. When the writer of the letter signs his name, it should also mention his job title (Head of Department, HR Manager etc.)
