So I kind of already know the answer but just to confirm and get any additional information regarding similar situation.

My PGWP expired as of today and BOWP is on hold. No update on PR application (AOR jan 9th) due to medical problem. Now I am require to do Biopsy and surgery and provide those reports to IRCC otherwise my medical will not pass, for me to do that in Canada is really expensive so I am planning to go back to my home country and have everything done there.

Since I am on implied status I know I won't be able to travel back to Canada,
but my concern is will it have any issue with my PR and BOWP application ?
Should I change the address so any future communication happen with home country address ?
Am I going to face any problem at airport for potentially staying in country for more then length of my visa ?

It will not affect your PR application but it will affect your BOWP application. If you have a valid temporary visa, you may be able to enter Canada. However, if not, most likely, your BOWP will be rejected and you will not be allowed to enter Canada. http://www.cic.gc.ca/english/resources/tools/temp/visa/validity/implied.asp

Huh, what a unique case. I think I would weigh up the cost of your flights home vs the cost difference on your medical procedure. Also something to consider could be the difference in time submitting - I wonder if the medical processing at home will be as quick to send off to CIC.

Couple of things to consider.

flight home is actually cheaper compare to medical procedure be done in Canada.

Also from my experience I would be able to finish everything quicker back home compare to in Canada because trust me when I say Medical system in Canada is really pathetic, It's free(well atleast for the most part), but it's really slow and pathetic.

So if there are no concerns with PR application I would go and do it back home.
