Is IMM1017 the Appendix C?

The checklist says IMM1017 as the physical exam form should be included in the application package.But when I telephone the panel physician they said it is Appendix C. Are they the same? thanks!

there is no IMM form for the medical anymore. the system is now electronic. you will be doing an "upfront" medical with the panel physician. at the end of your exam, the office will give you an "emedical" receipt which will have a barcode, your picture and identifying information. THAT is what is put in the package.

Thanks rhcohen 2014. I will do the exam tomorrow. They need color photocopy of my passport. Will the first page of my passport that has my personal information enough?

yes, that is what they need, a copy of the bio page of your passport.
