As a response to your inquiry, please note that if you wish to receive information for both you and your wife, it is recommended that you both sign consent forms showing that consent has been given to the individual you have appointed as you designated representative.

However, when you are applying for yourself (not through a representative) then I dont believe that your spouse (and I have NEVER send my spouse's consent) has to sign a consent form.
Now, yahoo7 ..............are you requesting for GCMS on your own or are you asking a representative to do it?
That is why I have mentioned in my previous post the question - are you requesting on your behalf? - if the answer to this question is yes, then you do not need a consent form signed by your spouse
But if, you are applying through a representative/friend/relative, then 1) you cannot answer "yes" to this question and, 2) you have to send a consent form signed by you and your spouse
I have asked similar question a few months back and did what some of the forum members suggested me.
