IRCC informed previously applied USA visa and got rejected but haven’t mentioned that in the application

IRCC send an E Mail to wife saying she previously applied for USA visa and got rejected and she haven’t mentioned that in the application.

But she never applied for USA visa previously, what can we do about it ?
