Our office strongly advises you to complete the application forms by computer and to generate a legible, 2D barcode by clicking on the “Validate” button at the top and bottom of the form. This will also perform an initial check to ensure that you have properly completed the forms.

i need your help po! pano ko po ba fifill-up yung mga forms , ipiprint ko yung forms then handwritten lng ang pagfill-up o online ko sya fifill-upan? pano po pg'online?

kabayan ano form b fill up mo. First download u muna file through PDF then fill in the computer then validate. Or easy way print u muna empty then complete the questions by hand then start again download then fill up the form you can save it muna before you validate to check properly the answers. After you review all the answers press validate then print. It won't validate if there's missing answers. Hope it helps.
