Inland Spouse PR Application Medical Request

Hello everyone. This is my first time posting here and unsure if these question have already been answered.

My wife applied for PR for my in DEC 2015 under the inland pr application. We were expecting turnaround in 24 months. Now after 14 months we have a request for a medical exam.

How long after the medical exam will get an update with my application?
Will I need to turn over my passport after a medical exam if I hold a USA passport?

Is it normal? shouldnt we get the Sponsor approval first (SA)? or AIP?

I have been reading from other threads that some people aren't getting AIP or sponsor approval. I recommend to link your application to Sign-In Partner and GCKey through CIC this will give you more up to date information about the progress of your application.
