Informations about father in IMM5645 | IEC.

Hey,
I have to give informations about my father in document IMM5654 for WH Work Permit visa, but I don't really know anything about him, only name and last name (and I'm not sure about that, he could change it, last time I had contact with him over 15 years ago).
Can I just leave it blank, or I've to send Letter of Explanation or something? I'll give informations about my mother and brother, but I'd like to leave "blank" informations about my father, cuz even if I find him or something, he would just lie about that, and I don't want to write something, which is totally uncertain.
What should I do?

Write what you know. Don't leave anything blank. If there's something you don't know - insert "unknown" or "n/a" into that field. Include a cover letter stating that you are estranged from your father and haven't had contact with him in 15 years - and therefore can't provide full details about him. They won't contact him. They won't contact your mother or brother either.

So I should write name and last name, then "unknown" about everything else, and add letter of explanation here? :

How long this letter has to be? Can I write it on computer, or it have to be handwriting + scan? And which informations should I put on that letter? My name, last name, something more? Signature?
Or can I just upload short note about that?
