Information required Background Declaration and Additional Family Form

Dear All,
I have Following Questions

1) I am adding my spouse as a dependent to my PR application. I have to send Schedule A: Background/Declaration (IMM 5669) for my spouse. In the last page where it says "Declaration by the Applicant", who will sign the declaration? Principal applicant or Spouse?

2) While filling Additional Family information for Spouse, should i write my name in Applicant or Spouse Name?

3) Who will sign Additional Family Information of Spouse?

Please help me as i have completed all the documentation requirements and will dispatch just after getting answers of these questions?
