judging by the email, your application has been directed to Paris visa office and you have received medical request,
here is what you need to do
1) goto cic website and find the doctors closest to your area(the link must be in the email). after you book an appointment with the doctor, you (and any co applicants) will be medically examined with blood, urine and xray tests. After completion of theses exams, they will give you a medical tracking sheet which you have to send(the actual sheet) along with scanned copies of your passport(s) to paris vo. you can also ask the doctor if you have confusion
2) again a link on how to pay this fee must have be provided within the email. I suggest you pay the fee via credit card as helps in moving you application at faster rate. If you pay via cc, you will be given receipt which you have to fill out and post with your documents to paris vo
3) this is easy to understand and self explanatory
4) you must get all the pcc for the countries the vo has asked you. again the instructions for getting pcc from all the countries mentioned in your email should be there in your email in the form of a link.

if you have any doubts, you can send me a pm

Hi,
